Evolved Body Art Minor Policy

Evolved Body Art chooses to accept minor clients on a case by case basis.  Each piercer or tattoo artist may have his or her own guidelines. Please feel free to call our piercing studio at 614-421-4444 to check with the piercer on staff that day or either studio at 614-421-4444, or 614-291-1505, to check with our tattoo artists.
Minors must provide:
state ID, driver's license, or passport
AND
birth certificate
AND parent's state ID, driver's license, or passport (parent must be present)

Also note:
If there has been a name change (ie, child's name does not match parent's), you must provide documentation of the name change.
